Current Scholarship Recipients: If you are attending a school different than the one indicated on your scholarship application, please fill out the School Change Form below and fax it to (507) 931-8033. You also need to complete a School Change Form if your enrollment status changes (from full-time to part-time or vice versa). Full-time status is usually 12 or more credit hours.
