To be eligible to apply for a Clara Abbott Foundation scholarship, you will need to submit a copy of your most recently filed IRS Form 1040 or planilla (U.S. - first two pages; Puerto Rico - first three pages and Schedule A1 for dependents). If your child is not listed as a dependent on your 1040 or planilla, please provide the 1040 or planilla of the person supporting the student. Even if the student is not listed as your dependent on your 1040 or planilla, we still need your 1040 or planilla in addition to one of the items below:

Student is:

• Natural child: residency documentation such as driver's license or ID card displaying address
• Natural child of divorced or separated parents: divorce decree or birth certificate
• Adopted child: adoption court papers
• Legal ward: court papers
• Stepchild: marriage license of stepparent and birth certificate of stepchild
• Domestic partner's child: notarized Abbott affidavit of domestic partnership

Please note that for all examples in the above information, the student must reside with or be primarily supported by the Abbott employee.
