Active employees
Eligibility to apply for monetary assistance begins 12 months after the date of hire (or one year from date of Abbott acquisition) for employees eligible to receive benefits and enrolled in a health plan (if applying for medical assistance).
Former employees
Terminated employees will receive assistance that was approved prior to the date of termination. Applications will not be considered if received after the date of termination (last day worked).
Survivors
Surviving spouses are eligible to apply until re-marriage. Surviving minors are eligible to apply until the age of 19 if not a full-time student or 23 if a full-time student. Special-needs children will not lose eligibility based on age. Employees on a disability program sponsored by Abbott are eligible to apply.
Retirees
Former Abbott employees are eligible for Foundation assistance as a retiree if the following requirements are met (at the time of separation from Abbott):
- 50 to 60 years of age plus a minimum of 10 years of service
- 61 years of age plus a minimum of 9 years of service
- 62 years of age plus a minimum of 8 years of service
- 63 years of age plus a minimum of 7 years of service
- 64 years of age plus a minimum of 6 years of service
- 65 years of age plus a minimum of 5 years of service
